We report a case of severe anaphylactic reaction in a 4-mo-old exclusively breastfed infant after ingestion of his mother's milk; she had eaten a few hours before. The patient had suffered from atopic dermatitis (AD) since he was 2 mo old. Family history was positive for atopy (mother: AD; a paternal uncle: severe fish allergy). The infant had been exclusively breastfed from birth and his mother followed a free diet. She had eaten fish during pregnancy, but gave it up on commencing lactation.
