Pulsed wave Doppler ultrasonography was by medial sagittal scan and high frequency transducer (7-4 Mhz) oriented through bregmatic window used to study anterior cerebral artery flow in neonates suspected to have PDA. Normal resistence flow was considered according to resistance index of Pourcelot the average value being 0.65 to 0.81. Brisk decrease in late diastolic velocity with value beyond 0.81 is strong evidence of systemic-pulmonary shunt or veno-arterious shunt in keeping with clinical suspect of relevant left to right shunt through PDA. Normal IR doesn't exclude high pulmonary pressure with bidirectional or right to left shunt.
