Erythema nodosum has long been recognized as a 'type reaction', occurring as an identical response to a wide variety of provoking agents and it's a rare complication of treatment with isotretinoin. Nowadays it's evident that erythema nodosum may represent an Arthus phenomenon to various antigens, such as Propionibacterium acnes, or a manifestation of circulating immune complex or an expression of a direct action of isotretinoin. The authors describe two cases of erythema nodosum, cropped up during a treatment with isotretinoin for a severe acne. Other significant clinical symptoms were pyrexia, arthralgia, leukocitosis. In both cases there were circulating immune complexes. Rapid regression of erythema nodosum and improvement of acne with suspension of isotretinoin and a systemic therapy with corticosteroids and antibiotics.
