Takotsubo syndrome or apical ballooning syndrome (ABS) is a transient acute coronary syndrome, usually following significant emotional stress. The coronary angiography is negative and the prognosis is good, with no need of prophylactic therapy. We describe a 73 year-old man who reported, following a domestic incident, a severe head injury (acute left subdural hematoma, surgically evacuated). In the following hours he developed ABS. After 6 months, right temporomesial complex partial seizures appeared and persisted despite carbamazepine. During a seizure he had a second episode of ABS indicating that this peculiar syndrome ma be a rare cardiac complication of seizures. In this way, it may be hypothesized that ABS might favour sudden unexpected death in epilepsy (SUDEP).
