Objectives Squamous cell carcinoma (SCC) of the lips is one of the most commonly occurring oral cancers, mainly involving the lower lip. Despite of a good prognosis in case of precocious detection, survival rates drops dramatically in presence of recurrence. Whereas many studies are present in literature regarding the management of primary tumors, the rate of recurrences of the lower lip SCC is low and therefore there are few data and articles on them. Study design Patients affected by recurrent SCC of the lower lip surgical treated from 2011 to 2019 in a single Center were retrospectively reviewed. Data and results were compared with literature. Results A total of 16 patients (mean age: 78,1 years) were eligible for inclusion in the study. The disease free survival rate at 2 and 5 years was 29,6% and the overall free survival rate at 2 and 5 years was 14,4%. A significant higher rate of failure and a worse prognosis was observed in patients with neck involvement. Conclusions The prognosis of recurrent lower lip SCC is extremely unfavorable with very low survival rates. This issue is probably linked to the oncological pathology but also to the advanced age of most patients, to the presence of several comorbidities and consequently to the high risk of peri-operative mortality. For this reason, the analysis of data set reported in the literature may help the surgeon in the management of recurrences and selection of the patients.
