[18F]FDG PET can be useful for the differential diagnosis within neurodegenerative parkinsonian syndrome (especially when an atypical parkinsonism is suspected). Here we present the case of patient with PSP showing hypometabolism in frontal cortex and anterior cingulate and in both putamen. This is the typical pattern expected at [18F]FDG PET in patients with PSP; however in this case, semiquantitative analysis allowed to identify more clearly the pattern as well as to highlight the presence of hypometabolism in the brain stem.
