Technological application has always been a critical theme in surgical improvements. Transanal surgery is a field where robotic platforms could be a game-changer. There is always more evidence that transanal excision of selected cancerous rectal lesions, if correctly performed, could be a valid and oncological safe alternative to more invasive traditional procedures. In this field, robotics could not only help improve the precision of surgical manoeuvres but, more importantly, also help spread some existing organ-sparing techniques whose application was always limited by intrinsic difficulty. The rising clinical need is leading to an exponential increase in platforms and robotics solutions to make transanal surgery more achievable and beyond its limits.
