The three-dimensional vascular anatomy of the spinal cord is intricate and attractive to understand. An appropriate knowledge of the vessels anatomy, as well as its embryological development, is the basis for the comprehension of the medullary vascular pathologies. In this chapter, we report an exhaustive description of the intrinsic and extrinsic spinal cord vascularization of the arterial and venous system. We indicate the most appropriate angiographic approach to reveal cervical, thoracic and lumbar spinal cord vasculature, focusing on the pathophysiological and clinical aspects.
