The paper examines inequalities in access to healthcare services between urban and rural areas in Italy, showing that these disparities stem from unequal resource distribution rather than natural differences. After distinguishing between environmental and spatial justice, the latter is proposed as the appropriate framework for understanding the issue. Rural communities, often marginalized, play a critical role in preserving territory and cultural heritage without adequate support. The paper calls for rethinking justice principles to include the spatial dimension, promoting policies that ensure equitable healthcare access and value the unique contributions of rural areas.
