Various syndromes are described in midbrain infarction. We describe a patient with a pontomesencephalic tegmental infarction with hearing loss and hypogeusia as dominant and persistent features. In stroke patients hearing loss is uncommon; in our patient the predominance of contralateral hypoacusia suggests that acoustic fibers are mostly crossed at the level of the inferior colliculus. Whereas the contralateral hemiageusia suggests the involvement of the crossed fibers of the secondary gustatory pathway at the upper pontine level.

Abstract
Various syndromes are described in midbrain infarction. We describe a patient with a pontomesencephalic tegmental infarction with hearing loss and hypogeusia as dominant and persistent features. In stroke patients hearing loss is uncommon; in our patient the predominance of contralateral hypoacusia suggests that acoustic fibers are mostly crossed at the level of the inferior colliculus. Whereas the contralateral hemiageusia suggests the involvement of the crossed fibers of the secondary gustatory pathway at the upper pontine level.File in questo prodotto:
