Abstract. Thirty years ago the Italian health care service was reformed and a National health service (Nhs) was introduced. This paper aims to evaluate the equity impact of the Nhs. Inequalities in mortality and morbidity (disadvantaging the Southern regions and the deprived population) have not changed during these 30 years, mainly caused outside the health care sector. The resource allocation and utilization of care seem substantially proportional to the need (age and deprivation). However when analysing specific courses of care, substantial social and geographical inequalities in access to timely and appropriate interventions and in their outcomes come is shown. The paper discusses the policy implications of such inequalities in health and health care. Key words. Essential health care services, geographical inequalities, policies, social inequalities.
