This study evaluated whether a single implant–retained mandibular overdenture can improve sensory discrimination and masticatory performance in edentulous patients over a 10-year period. Thirteen patients (mean age 67) received conventional complete dentures followed by the placement of one mandibular implant and delayed loading with Locator® attachments after three months. Thickness discrimination thresholds and masticatory efficiency were measured five times: before implant insertion and at 3 months, 1 year, 6 years, and 10 years after prosthetic connection. Radiographs monitored peri-implant bone levels at multiple intervals. Three patients were lost to follow-up due to death. Results showed significant improvement in thickness discrimination and chewing ability during the first year, with stability maintained throughout the 10-year observation period. No implant failures occurred, and mean peri-implant bone loss reached 1.34 mm at 10 years. Prosthetic maintenance requirements were consistent with existing literature. Overall, the single implant–retained overdenture represents a conservative, effective, and suitable option, particularly for frail patients.
